The Kitty Hawk incident on the 84' Westpac and 3 weeks in Hawaii replacing the #1 Shaft that the roveing watch never checked. The only place where you can screw up and cost the Govt millions of $ and get a slap on the wrist. I think he got RIR from PO3 to FN and 2 months extra duty and restriction and a $450 fine.
